Citizens Advice Wandsworth is an independent charity that provides information, advice and support. We also work to improve the policies and practices that affect people's lives. We accept referrals for: - Welfare benefits advice: Form filling (disability benefits), challenging decisions and completing SSCS forms. We cannot attend tribunals. - Housing: Possession claims, homelessness, eviction. We do not attend hearings. - Immigration: We can arrange a free 30 telephone minute appointment. We can also provide assisted information and signposting. We do not currently accept referrals for debt. Debt cases can be referred to South West London Law Centre. We aim to accept referrals within 2 working days of receiving them. When we accept a referral, we place the client in a queue to be contacted. If the issue is urgent (eg. foodbank voucher required or imminent eviction) we will contact the client as soon as possible. For non-urgent issues, we will aim to contact the client within 10 working days. Please explain our response times when referring a client.

CARAS is a community outreach charity based in Tooting. We support people of refugee and asylum-seeking background who live in South-West London. We have social activities (coffee mornings, sports, trips, yoga, youth club), English classes (at all levels) and a project for newly arrived children waiting for school places. Our services are for both for young people (14+) and adults (18+). Some of our activities are for female-identifying people only.

Welcare is a registered charity that works to provide practical and emotional help and support to families who are experiencing difficulties. Our South West London service offers support for families with children up to 13 years old. We work with the whole family, providing non-judgemental, honest advice and guidance, helping to empower families to overcome the challenges they face. Our SW London office supports families residing in Wandsworth and Richmond boroughs. We also facilitate an education programme for parents, Strengthening Families, Strengthening Communities and My Space groupwork for children who have been impacted by Domestic Abuse. Referral Criteria and Pathways We take referrals from schools, churches, children’s specialist services, Health Visitors, GPs and other professionals; children centres; other voluntary organisations. Families can also self refer for support Referrals are accepted where there are children aged 0-13 yrs. Families with older children may be considered for support. Description

Racism, discrimination and inequalities impact many of our fellow citizens and neighbours, pushing them to the margins of our communities and creating barriers that limit their abilities to live full lives. This affects us all, weakening our civic morality, and our trust and confidence in each other. Much of the responsibility for addressing structural inequalities and systemic racism has fallen on our public institutions, who in some cases are responsible for sustaining these fault lines, and therefore are not equipped to bring about the changes that we want to see. Bringing about a society that is just, equal, fair and anti-racist, is our collective responsibility. The biggest resource that we have to achieve this vision is ourselves. We are the people that we have been waiting for! We choose to stand in the space between active citizens and communities and public institutions, facilitating collaborative, coproduced solutions and interventions that address some of our most complex challenges. Presently, we are tackling health inequalities, and from there we move forward.

Family Action

Description

The Wandsworth Well Family Service is an early intervention, mental health and well-being service based in a number of GP practices across the London borough of Wandsworth. The service is a frontline source of help for adults with mental ill-health who present to GPs with a wide range of additional and often complex, non-medical support needs or social problems. This may include problems with housing, finances, and employment or family relationships. The service is currently based in six GP practices in Wandsworth, and builds on the successful approach taken in the London Borough of Hackney. An evaluation of that service previously undertaken showed reductions in GP attendances by up to 75%.1, the service aims to overcome stigma and discrimination regarding mental health issues, through a holistic recovery approach, and by providing information at the point of referral and support for people to lead an active life in their community. The service works alongside more specialist mental health services locally where relevant, including community mental health teams and psychological therapies (IAPT), and support workers refer to these and other agencies as required. This may include employment support, peer networks, volunteering, community groups and other statutory services. Important note: we are a GP based service and it is always helpful to have details of the service user's GP at the point of referral. Registration with a Wandsworth GP is a requirement for referrals into the WellFamily 6 session model.

Healthwatch Wandsworth is an independent consumer champion that gathers and represents the views of people who use local health and social care services on Wandsworth. We present your views in reports and make sure those who have the power to change things hear what you had to say. Health and social care services have a legal duty to respond to our recommendations and requests for information. We find out what you think about services by:             - Visiting local community groups and events.             - Visiting services and speaking to patients.             - Hosting events, our newsletter, website and social media pages. WRAP - Thinking Works

Thinking Works is a not-for-profit organisation specialising in advice services that help households stay warm for less. In Wandsworth we manage the Wandsworth and Richmond Assessment Point (WRAP) service. WRAP offers a free home visiting service offering advice on fuel bills, applying for the Warm Home Discount, advice on using heating systems efficiently, advice on damp and mould, and assessments for heating and insulation grants (when available). The service also includes the installation of free small measures such as LED bulbs and radiator reflector panels for eligible households to help lower fuel costs.  The WRAP service is free and open to referrals from the following groups: *People with a low income *People aged 65 or over *People with a long-term condition such as diabetes or COPD *People registered as disabled *People with children under 5 in the household *Anyone identified in need and referred by a third-sector organisation such as CAW or Age UK Wandsworth

The Wandsworth Wellbeing Hub is a free signposting service that helps support Wandsworth residents to find local services to meet their needs. It can help people with a variety of issues, from individuals who wish to access local counselling services, get better involved in the community, manage their weight etc. We also act as an information point for additional services provided by charities, self-help groups and voluntary sector organisations in Wandsworth. Referrals can be made for anyone aged 18+ who lives in Wandsworth, works in Wandsworth and/or is registered with a Wandsworth GP

SPEAR London

Description

The SPEAR Outreach team operate in the Wandsworth borough to support rough sleepers. Our team go out late at night and in the early hours of the morning to identify those who find themselves rough sleeping with the objective of verifying them as rough sleeping and helping sign-post those rough sleepers to relevant services. If you are concerned about a rough sleeper and want SPEAR to look for them on shift, we ask you to please refer them to us via refernet. Please be as specific as possible in relation to the location of the rough sleeper to enable us to provide Outreach services effectively. The Outreach team are on shift twice a week and endeavor to respond to referrals within 72-hours. Please note, rough sleepers must be verified by SPEAR in order to be assigned a keyworker to receive support. Verification means they must be seen bedded-down by our Outreach service whilst out on shift late at night or in the early hours of the morning.
