Description

We have two adult mental health services serving Wandsworth. Please specify which service you are referring to. Both offer phone or face2face support. • The Primary Care Integrated Wellbeing and Support Service This is open to people who are registered with a Wandsworth GP. We provide an assessment and two sessions of follow up support. Referrals can also be made via our Family Action website using our link: Wandsworth PCI Wellbeing and Support Service We accept self referrals. Face2Face support takes place at the GP surgeries. • Wandsworth Wellbeing Foodbank Service. This is open to the Wandsworth community including Wandsworth Foodbank and Earlsfield Foodbank guests. Self referrals can be made by contacting our service. We provide an assessment and two sessions of follow up support. Face2Face support takes place at the foodbanks or other community venues. Details of both services: Both are early intervention, mental health and well-being services located in the heart of the community including GP practices and Foodbanks across Wandsworth. The services are a frontline source of help for adults with mental ill-health who present to GPs and community venues with a wide range of additional and often complex, non-medical support needs or social problems. In our support of adults we work holistically with the whole family. Tmay include problems with housing, finances, and employment or family relationships. Through inclusive and personalised support, we aim to improve the emotional health and wellbeing of individuals and impact local families, young people, children and the community. Referrals: The services are open to over 18's and both provide early help support and therefore not suitable where there is a crisis or safeguarding at the point of referral. The referral criteria for each service are slightly different. Please specify which service The support is short term, face 2 face or by phone and includes an assessment, help to access other community and specialist services as well as providing information, advice , guidance and emotional support.

Samuel’s Charity was founded in 2014 by Martin and his son, Samuel, who sadly passed away at the age of nine after being diagnosed with a rare form of leukaemia. Samuel's Charity is committed to improving paediatric hospital discharge processes and supporting essential at-home care to support the whole family. Samuel’s Charity is dedicated to supporting efficient paediatric hospital discharge and at-home care. Through lived experience, Martin understands the significant impact prolonged hospital stays can have on patients and their families/carers. The organisation collaboratively works with St. George’s Hospital in Wandsworth, the NHS paediatric nursing community team, and the wider charity and not-for-profit sector to ensure children receive the in-hospital support they need to facilitate efficient discharge and at-home support to minimise readmissions.

Healthwatch Wandsworth is an independent consumer champion that gathers and represents the views of people who use local health and social care services on Wandsworth. We encourage those who design and run health and social care services to find out what you think, and to involve you in improvements and changes. We find out about major changes to services to make sure that the people who use them are listened to and understand the changes. We present your views in reports and make sure those who have the power to change things hear what you had to say. Health and social care services have a legal duty to respond to our recommendations and requests for information. We find out what you think about services by: - Visiting local community groups and events. - Visiting services and speaking to patients. - Hosting events, our newsletter, website and social media pages. We also provide information and signposting to help you find the local health, social care and support services you need, help you understand how to make a complaint, or find advocacy services that could support you with your complaint.

Citizens Advice Wandsworth

Citizens Advice Wandsworth is an independent charity that provides information, advice and support. We also work to improve the policies and practices that affect people's lives. We accept referrals for: - Welfare benefits advice: Form filling (disability benefits), challenging decisions and completing SSCS forms. We cannot attend tribunals. - Housing: Possession claims, homelessness, eviction. We do not attend hearings. - Immigration: We can arrange a free 30 telephone minute appointment. We can also provide assisted information and signposting. We do not currently accept referrals for debt. Debt cases can be referred to South West London Law Centre. We aim to accept referrals within 2 working days of receiving them. When we accept a referral, we place the client in a queue to be contacted. If the issue is urgent (eg. foodbank voucher required or imminent eviction) we will contact the client as soon as possible. For non-urgent issues, we will aim to contact the client within 10 working days. Please explain our response times when referring a client.

Welcare is a registered charity that works to provide practical and emotional help and support to families who are experiencing difficulties. Our South West London service offers support for families with children up to 13 years old. We work with the whole family, providing non-judgemental, honest advice and guidance, helping to empower families to overcome the challenges they face. Our SW London office supports families residing in Wandsworth and Richmond boroughs. We also facilitate an education programme for parents, Strengthening Families, Strengthening Communities and My Space groupwork for children who have been impacted by Domestic Abuse. Referral Criteria and Pathways We take referrals from schools, churches, children’s specialist services, Health Visitors, GPs and other professionals; children centres; other voluntary organisations. Families can also self refer for support Referrals are accepted where there are children aged 0-13 yrs. Families with older children may be considered for support. WRAP - Thinking Works

Thinking Works is a not-for-profit organisation specialising in advice services that help households stay warm for less. In Wandsworth we manage the Wandsworth and Richmond Assessment Point (WRAP) service. WRAP offers a free home visiting service offering advice on fuel bills, applying for the Warm Home Discount, advice on using heating systems efficiently, advice on damp and mould, and assessments for heating and insulation grants (when available). The service also includes the installation of free small measures such as LED bulbs and radiator reflector panels for eligible households to help lower fuel costs.  The WRAP service is free and open to referrals from the following groups: *People with a low income *People aged 65 or over *People with a long-term condition such as diabetes or COPD *People registered as disabled *People with children under 5 in the household *Anyone identified in need and referred by a third-sector organisation such as CAW or Age UK Wandsworth

SPEAR London

Description

The SPEAR Outreach team operate in the Wandsworth borough to support rough sleepers. Our team go out late at night and in the early hours of the morning to identify those who find themselves rough sleeping with the objective of verifying them as rough sleeping and helping sign-post those rough sleepers to relevant services. If you are concerned about a rough sleeper and want SPEAR to look for them on shift, we ask you to please refer them to us via refernet. Please be as specific as possible in relation to the location of the rough sleeper to enable us to provide Outreach services effectively. The Outreach team are on shift twice a week and endeavor to respond to referrals within 72-hours. Please note, rough sleepers must be verified by SPEAR in order to be assigned a keyworker to receive support. Verification means they must be seen bedded-down by our Outreach service whilst out on shift late at night or in the early hours of the morning.
